Care coordination
Care coordination helps clients identify and access the services that will best address the problems they are experiencing. The care coordinator may also assist a client in developing the skills that help them learn how to use the resources and services available to them independently.
Targeted Case Managers
Our care coordination services use Targeted Case Managers to assist active clients at our Malone or Saranac Lake Behavioral Health Clinics. These case managers assist clients with any additional needs or concerns outside of the counseling services they receive through the clinic. Clients can request these services through their therapist.
Health Home Care Coordinators (HHCC)
Our care coordination services also include Health Home Care Coordinators. The coordinators assist clients struggling with multiple disorders (physical, mental health, and/or substance abuse) so that they may function at a higher level. We offer HHCC services to both children and adults. In addition, we offer an assisted outpatient treatment care coordinator to qualifying individuals. This form of care coordination is authorized through a voluntary service agreement or the direction of a court-ordered psychiatrist overseeing the client. Individuals who have two or more psychiatric hospital admissions and/or incarcerations that relate to a risk-of-harming others are qualified for this program.
HHCC can be requested by an individual, family member, or service provider through a referral or by asking for assistance from a therapist or Targeted Case Manager. The individual or the client must agree to services before enrollment since this is a voluntary program. Referrals should be directed to the supervisor of health home services.
