Job Coach

Position Details:

A Job Coach works with people who receive employment services/supports through Career Visions. The job coach is there to support the person at their job, assist them with learning all the new tasks associated with their job and assess their current skill level. The job coach also acts as a role model/mentor showing the person all the various skills necessary to be successful in competitive employment. The ideal candidate for the position will have a positive attitude, be able to work independently and have the ability to teach and motivate others.

High school diploma or equivalency, and a valid NYS drivers license. Experience working with people with disabilities preferred, but not required.

LPN - Part Time

Position Details:

​Citizen Advocates is seeking permanent and per diem Licensed Practical Nurses to fill positions for medically frail residential setting. Several shifts available. Applicants must possess competent nursing skills and be able to work well with others. Duties will include administrating medications, performing routine nursing tasks and providing patient care to disabled adults. Duties may vary per operational needs. Experience with Feeding tubes preferred but will train. Compassion and caring a must. Must be able to lift 50 lbs. 12am-9am Friday, Saturday and Sunday (27 hours), 3:15pm-12:15am, Friday and Saturday (16 hours) Per diem hours available.

Child Health Home Care Coordinator - Saranac Lake

Position Details:

We are looking for an experienced candidate who wishes to make a difference in the lives of children and their families coping with the impact of mental and/or physical illness. Our Health Home program is seeking enthusiastic candidates, who have experience providing case management and working with children and families to join a fantastic team of care coordination staff. This Saranac Lake position will work with youth and their caregivers to identify goals, explore ways to achieve them, and support and link the clients to resources to make these dreams a reality. Care coordinators maintain regular contact with clients and their service providers to provide support and coordinate care. Ability and desire to work with a diverse client population, strong written and verbal communication skills, and effective time management are a must.

Controller

Position Details:

​Ensures accurate books and records, payor, grant, regulatory reporting, “a no surprises control environment” and associated analysis through the management of timely financial cycles, such as revenue, expense and capital accounting. Additionally, manages: i) financial transaction processes in payroll, treasury and purchasing, ii) financial audits, including the financial statements, payor, regulatory and grant requirements, iii) the production of the annual audited financial statements, iv) the production of monthly, internal financial reports (actual results) while working collaboratively with financial planning and budgeting to produce the final reports and associated analysis, v) purchasing and sourcing, vi) the growth and professional development of the team vii) provides insights and recommendations on accounting policy, control structure and opportunities to improve financial efficiency and service levels. Reports to the Chief Financial Officer.

Qualifications:

Bachelor’s Degree in accounting, finance or related field. 10 years of progressive experience in financial management with technical capabilities in accounting, cost accounting, US GAAP, Internal Revenue Code, Regulatory Account, Financial Technology and transaction. Must be an effective and collaborative leader within and outside of the department to ensure an effective control environment. Experience in a healthcare environment a plus.

Afterhours Behavioral Health Clinician

Position Details:

Providing behavioral health intervention and stabilization services within an on site multi disciplinary team.

Crisis Worker - Per Diem

Position Details:

Providing mobile behavioral health crisis intervention, afterhours and on weekends in Franklin County. Base of operations is your home, must be available during shift at all times and must be able to travel as needed. Attendance at bi-weekly team meetings required (may be by phone). Position entails one night every seven weeknights and one weekend every seven weekends. Sixty minute, bi-weekly team meetings required (may be by phone).

Targeted Case Manager

Position Details:

The Targeted Case Manager (TCM) Program is seeking well-organized individuals who have a passion for helping clients coping with life challenges to obtain the support they need to achieve their goals. This Saranac Lake position will work with clients seeking behavioral health services to identify additional needs and assisting them in connecting to other agency and community resources. Successful TCM staff adapt their approach to engage clients with different backgrounds, styles of commutation, and needs. They will enjoy creative problem solving and an ability to work independently while also actively contributing to their team of coworkers. This is an ideal position for individuals who have strong interpersonal skills that wish to gain additional experience working with clients with mental health and substance use disorders.

Direct Support Professional - Saranac Lake

Position Details:

Residential:
​Assisting persons with developmental disabilities in a residential setting. Responsibilities include assisting residents with daily living skills, community integration, cooking, cleaning, bathing, and daily documentation of programming. Flexible hours including evenings, nights and weekends.

Day Program:
Qualified candidates must be energetic, caring, creative, and enjoy being out in the community. Candidates will be expected to encourage independence with activities and offer choices that the people they support find meaningful and valuable. Other duties include but not limited to, providing daily therapeutic and skill building activities, recording daily documentation of services, and working as a team member with other Direct Support Professionals to provide the highest quality of care and services. Monday - Friday daytime hours

At Home Waiver Program:
​Provide community habilitation, day habilitation, and respite services to people living at home. Provide services which promote independence according to the person's interests and goals. Provide services in the home and in the community. Flexible hours.
