Direct Support Professional - Saranac Lake

Position Details:

Residential:
​Assisting persons with developmental disabilities in a residential setting. Responsibilities include assisting residents with daily living skills, community integration, cooking, cleaning, bathing, and daily documentation of programming. Flexible hours including evenings, nights and weekends.

Day Program:
Qualified candidates must be energetic, caring, creative, and enjoy being out in the community. Candidates will be expected to encourage independence with activities and offer choices that the people they support find meaningful and valuable. Other duties include but not limited to, providing daily therapeutic and skill building activities, recording daily documentation of services, and working as a team member with other Direct Support Professionals to provide the highest quality of care and services. Monday - Friday daytime hours

At Home Waiver Program:
​Provide community habilitation, day habilitation, and respite services to people living at home. Provide services which promote independence according to the person's interests and goals. Provide services in the home and in the community. Flexible hours.

Qualifications:

High school diploma, or GED, with experience working with the disabled population and ability to drive agency vehicles is a plus. Training provided for non-experienced applicants.

Clinicians - Saranac Lake

Position Details:

​Citizen Advocates is seeking motivated therapists to join our dynamic team at our cutting edge, co-occurring community mental health centers in Saranac Lake, NY. We are the fastest growing provider of treatment for mental health and substance use disorders in the north country. We provide the full range of behavioral health clinical services including comprehensive assessments, crisis intervention and same day services to individuals, groups and families. Supervision toward obtaining clinical social work or mental health counselor licensure will be provided as well as numerous opportunities for growth and advancement. We offer a highly competitive salary and benefits package. Federal student loan repayment program is available to eligible staff.

Qualifications:

Habilitation Coordinator

Position Details:

Completion of written plans and monthly notes for individuals receiving Home and Community Based Waiver Services. Attendance and participation at person centered planning meetings. Monitoring of written plans and documentation of services. Maintenance of clinical record and program books. Communication with families and outside agencies.

Weekly Hours:

​​40 hours per week, Monday-Friday 8-4:30 (some flexibilty required)

Qualifications:

​B.A. in Human Services or related field and one year experience with individuals with developmental disabilities.

Customer Support - 2 Part Time Positions (Saranac Lake)

Position Details:

Customer Support positions available in our Saranac Lake, NY outpatient clinic. We’re looking for highly motivated, self-directed, organized individuals. Computer literacy and multi-tasking required for our high patient volume environment. Duties include but are not limited to daily Customer Support duties such as patient scheduling, managing provider schedules, answering/directing phone calls and communicating with multiple agencies and community members. Proficiency in Microsoft Excel and Adobe Acrobat is a plus.

20-25 Hours/Week

Qualifications:

High School Diploma or GED, proficiency in Microsoft Excel and Adobe Acrobat is a plus. ​

Greenhouse Worker

Position Details:

Will perform general greenhouse duties--watering, planting, transplanting, plant maintenance, etc. Will assist in general maintenance of facilities.

20-28 hours schedule will vary with season

Qualifications:

High School diploma/GED or 2 years experience with job related duties. May require lifting of up to 50 pounds.

Compensation and Benefits:

Program Manager

Position Details:

Overall administrative responsibilities of HCBS/Waiver Services.

•Oversight of the programmatic and administrative operation for the Clinton County At Home Waiver Program in accordance with current Federal, State, and local standards, guidelines, and regulations that govern service delivery.
•Completion of documentation, reviewing of billing and payroll, regular review of program books
•Hiring and oversight of direct support professionals, ensuring that staff are meeting expectations
•Ensuring individuals we support are receiving quality care
•Communication with outside parties including Care Coordinators and families

Full Time

Weekly Hours:

40 hours, M-F 7:30-4

Qualifications:

Bachelor's Degree in Human Services, Business Administration or related field with one year experience, or Associate's Degree in Human Services or Business Administration with five years related experience.

Licensed Practical Nurse

Position Details:

LPN position available at a medically frail residential setting. Applicants must possess competent nursing skills and be able to work well with others. Duties will include performing routine nursing tasks and providing patient care to disabled adults. Duties may vary per operational needs. Experience with Feeding tubes preferred. Compassion and caring a must. Must be able to lift 50 lbs.
